Definition
Fee Schedule refers to the payer- or provider-defined list of allowed reimbursement amounts for specific procedure codes (e.g., CPT/HCPCS or CDT) and applicable modifiers. It drives the allowed amount used in claim adjudication and patient cost-sharing calculations. Example: a payer may allow $92 for CPT 99213 in-network, requiring the provider to adjust any billed charge above that amount down to the contractually allowed level.
Why it matters
At enterprise scale, outdated or inconsistent fee schedules create systemic underpayments and inaccurate patient estimates. Even a 1% underpayment variance on $80M in annual payer receipts equals $800,000 in leakage; manual maintenance across 100+ locations can also consume 40–80 labor hours per payer refresh. Tight fee schedule governance protects margin and reduces rework across DSOs and health systems.
