Definition
Contracted Rate is the negotiated in-network allowed amount a payer agrees to reimburse a provider for a specific service, before patient cost-sharing is applied. It governs what the payer will pay and what the provider must adjust off relative to billed charges. Example: if the contracted rate for a preventive visit is $140 and the practice bills $200, reimbursement is based on $140 minus deductible/coinsurance, not $200.
Why it matters
For multi-location providers, small variances between posted payments and contracted rates compound quickly. Detecting a 2% average underpayment across $60M in net payer revenue recovers $1.2M annually; incorrect rates can also distort patient estimates and add 5–10 A/R days. Centralized oversight of contracted rates is core to revenue integrity and payer performance management.
